Privacy Complaints or Requests to Review Responses to Access Requests

  • Request for Review and Privacy Complaint Form: This link takes you to the Request a Review / File a Complaint webpage. We ask you to read the information on that webpage before downloading, completing and submitting the Request for Review or Privacy Complaint Form to the OIPC. The form link is available at the bottom of the Request a Review / File a Complaint webpage.
  • Change of Contact or Address for Service: This form is intended to update contact information or address for service for a specific case file only. A copy of the completed form must be exchanged between the public body, custodian or organization and all affected parties. If an individual or public body/custodian/organization wants to update their contact information or address for service for all of their case files, please send a letter to the OIPC. Do not use this form if you are wanting to update your contact information or address for service for all case files.
  • Authorization to Represent in a Review: This form is meant to authorize another individual to represent you in a request for review or privacy complaint.

Forms for Use by Public Bodies under the Freedom of Information and Protection of Privacy Act (FOIP Act)

  • As of June 2024, there are two forms for public bodies to submit time extension requests under section 14 of the Freedom of Information and Protection of Privacy Act (FOIP Act).
  • Please note this important process change - As of February 1, 2024, the OIPC will no longer be conducting courtesy searches on behalf of public bodies to determine if a third party request for review has been received by this office. The provision of this service is not a responsibility of the OIPC under the FOIP Act and as our office works to streamline and improve our processes to provide better service to Albertans in the areas of our legislated responsibilities, the provision of this courtesy service is being discontinued. Sections 30 and 31 outline the statutory obligations of public bodies and third parties under the FOIP Act. Public bodies and third parties should communicate directly with each other respecting these statutory obligations. In their written notice, public bodies may advise third parties that if the third party decides to ask for a review under section 31(3), and if the third party does not want to risk inadvertent disclosure of information by the public body, that third party should directly notify the public body that it has asked for a review.

PIPA Forms

These forms are meant to assist individuals who would like to request access to or correction of personal information held by a private sector organization. Do not use these forms for submissions to the OIPC.

Inquiry Forms

Please note, as of April 1st, 2024, the OIPC will no longer be using a Request for Inquiry form.  As such, this form has been removed from our website. If some or all of the issues in a request for review or privacy complaint have not been settled at the case resolution phase and an applicant/complainant wishes to proceed further in our processes, they must speak with the investigator (known as a Senior Information and Privacy Manager or SIPM) who dealt with the review or privacy complaint. The SIPM will then bring forward the unsettled issue(s) to the Commissioner to determine whether an inquiry will proceed on the unsettled issues.  

  • Undisclosed Party Change of Contact or Address for Service: This form is intended to update contact information or address for service for a specific case file only. For an affected party to update their contact information or address for service for all of their case files, please send a letter to the OIPC. Do not use this form if you are wanting to update your contact information or address for service for all case files.

Speaking Engagement Requests

Complete and submit the above form by email. The OIPC recommends that you submit a speaking engagement request at least three months prior to the event, to allow enough time for our office to review and respond to your request. Factors in our decision making include expenses, time commitment, size or type of the audience, and topic.
