On May 3, 2011, a one term insurance policy file was sent via courier to the wrong address. The breach was caused by selecting the incorrect receiver from the FedEx Shipping system. The incident was discovered on June 6, 2011 when the recipient who had received the file in error contacted the Organization to report the error. The file was sent to the recipient?s address, but was not addressed to the recipient. The recipient opened the package despite the fact it was not addressed to her in name. The package was recovered by the Organization.
